The Social Services Department has a broad area of responsibility that includes regulatory activities and preventive measures, with a reception and investigation unit, outpatient care, family law, financial assistance, and an operational unit working with preventive measures.
JOB DESCRIPTION
We are now looking for a Case Manager to work in the project 'steps towards self-sufficiency'. The project aims to provide residents who are far from the labor market with coordinated and individualized support to approach self-sufficient employment. The project applies an evidence-based working method, case management. The method consists of labor market rehabilitation measures to bridge barriers and gradually enable participants to find work or start studies.
As a Case Manager, you will focus on creating contacts with employers who can hire and work to connect participants with employers.
Your role includes supporting both participants and the employers who receive individuals to enable a lasting employment.
You will collaborate internally with colleagues and services as well as externally based on the needs of the work.
QUALIFICATIONS
We are looking for someone with a degree in social work or another academic qualification that the employer deems suitable for the position.
You have experience working with individuals who need coordinated support. It is an advantage if you have experience with evidence-based working models such as Case Management, Supported Employment, or BIP.
The position includes visits to companies, so you need to have a valid driver's license (B).
You are structured, goal-oriented, results-driven, and have good collaboration skills. You are confident in supporting individuals with different limitations. You instill hope and belief in their abilities to overcome their barriers to work or study. You have the ability to persuade and support employers to accept and try participants in their companies.
