Job Responsibilities
As a summer substitute social worker in our care planning team, you will be responsible, together with your colleagues, for authority exercise according to the Social Services Act (SoL) in connection with discharge from inpatient care and for follow-up of short-term decisions. The assignment includes investigating individual needs, making well-founded decisions, and planning interventions that ensure a safe and functioning care and support situation after hospital stays or short-term accommodation.
The work encompasses the entire process from needs assessment and decision to care planning and documentation in accordance with applicable legislation and guidelines. You will work both independently and in close collaboration with the team and cooperate with other professional groups within health care, relatives, and external actors.
We work both on-site at the office and travel around the city by bike or public transport to meet the target group where they are located.

Qualifications
It is required that you:
- Are a trained social worker
- Alternatively, are in training to become a social worker and have completed your internship (term 5) with a passing result of at least 120 credits (you will then be employed with the title administrative officer)
- Have good knowledge of applicable legislation
- Find it easy to understand instructions and can independently document in a secure and understandable way
- Can express yourself well in the Swedish language in both speech and writing
It is meritorious if you:
- Have completed your internship in authority exercise or have work experience in authority exercise
- Have previously worked with the elderly target group
- Have experience and knowledge of the operational systems Lifecare, Mina planer, or Procapita
To succeed in the role, you need to have good collaboration skills and enjoy working together with others towards common goals. You need to be able to think clearly, respond calmly, and maintain focus even when the situation is difficult.
We see that you take responsibility for both your own and the organization's and team's commitments, both when things go well and less well. The ability to prioritize and sometimes re-prioritize tasks when the situation requires it is also important in the role. You pay attention, energy, and resources to tasks and issues based on how important or urgent they are.
About the Workplace
The authority department in the Health, Care, and Social Services Administration consists of about 190 employees divided into three units: SVU/short-term, regular housing, and the unit for fees, mediation, and housing. We have several different professional categories gathered within the authority: managers, social workers, nurses, occupational therapists, physiotherapists, administrators, and development secretaries. Since January 2020, we have been in newly renovated premises centrally located in Malmö, Storgatan 20, with good public transport connections.
