We are looking for someone who is eager to continue developing the mobile team for primary care in central Skåne. You are welcome to apply for this exciting and developing position!

The mobile primary care team operates throughout Skåne and is perceived very positively by both patients and staff. In October 2024, we launched the team in Lund, covering the areas of Lund, Eslöv, Burlöv, Staffanstorp, Kävlinge, Lomma, Höör, and Hörby.

The mobile primary care teams represent a cohesive form of care where primary care is the hub in collaboration with other healthcare providers. The team offers safe, clear, and accessible care for patients who cannot reach a health center and who require medical care that is better provided at home than in a hospital. The team can assess, plan, prescribe, and perform interventions. Care is specialized based on each individual patient's needs, and the goal is to offer patients qualified care both medically and psychosocially.

We collaborate with SUS Lund, Kristianstad Central Hospital (CSK), ambulances, private and public health centers, and other healthcare actors in the area, such as the care and social services in each municipality.

The Department of Close Care and Health is under construction and will be operational from January 1, 2025. The department is responsible for grant-funded healthcare activities within close care. This includes mobile teams and specialized palliative care, outpatient services within close care, acute care within close care, 1177 by phone, and certain region-wide functions. Additionally, the department is responsible for the development and follow-up of the transition to close care in Region Skåne.

The team is based in Lund and consists of a unit manager, doctors, nurses, social workers, and medical secretaries.

RESPONSIBILITIES

Your position involves working in a team where you conduct home visits to patients in various municipalities. You lead and actively participate in planning around the patient's care together with other professionals and collaborate with other healthcare providers. In cases where patients may require hospital care, we strive for direct admissions to the ward. Participation in the development and improvement work of the organization is also part of your responsibilities.

Working hours are from 08:00 to 17:00, Monday to Sunday.

QUALIFICATIONS

We welcome you who are a licensed physician authorized to practice within Swedish healthcare and have specialist competence in emergency medicine or geriatrics. Language skills in Swedish corresponding to at least level C1 according to the Council of Europe's scale are required. A valid B driver's license is also required as care is largely provided in patients' homes. Experience from similar work is an advantage.

You who apply are secure in your professional role and have the patient in focus. Furthermore, you have the ability to see the individual needs of patients and a flexible approach to solving the daily work layout. Part of the work involves support and conversations, which requires you to be communicative and able to meet people at different stages of life. Quality and improvement work and close collaboration with the team are a given. Personal suitability is highly valued.
