JOB RESPONSIBILITIES

The Department of Health, Care, and Prevention consists of home care and home health care. To find the best individual solutions, we work in interdisciplinary teams. The focus of our development work is on digital solutions that simplify everyday life for employees and help ensure good quality for citizens.

As a dementia nurse with us, you will be part of our management team alongside unit managers and will have close contact with MAS/MAR.

In 2026, we will be one of the first municipalities to be Star Rated in all operations including authority, special housing, home care, and LSS. A significant and important investment.

Today, there is also a very good collaboration with the healthcare centers we work with, where you, as the role bearer, will be a key person regarding the care of those with cognitive impairment.

The role of the specialist nurse/Silvia nurse involves functioning as a consultant within the municipality and having a holistic perspective where the work area includes, among other things: Collaboration, Education, Supervision, Knowledge bearer, Community informant, Inventory and evaluation, as well as Individual support.

The role of the specialist nurse/Silvia nurse includes:

• Being part of the Cognitive specialist team in the municipality
• Being a guide for individuals with cognitive diseases and their relatives throughout the disease process.
• Providing support and advice to the operations in Burlöv municipality for counseling, supervision, and education on issues related to cognitive diseases.
• Working with family support and in close collaboration with the municipality's family coordinators.
• Being a face in the community that citizens can turn to with questions about cognitive diseases.
• Collaborating internally and externally for a better and smoother care chain for individuals with cognitive diseases.
• Being a knowledge bearer in the organization, staying updated on new research, etc., to maintain quality in care and support.
• Working on development issues based on the National Guidelines from the National Board of Health and Welfare for the care and support of individuals with cognitive diseases/dementia and other governing documents.

QUALIFICATIONS

Nurse with specialist training focusing on cognitive diseases or Silvia nurse.
