Mission

We are now looking for a colleague for our rehabilitation team, where you will be responsible for the SVU process and be part of our newly started homecoming team. The rehabilitation team is characterized by a good atmosphere and collaboration skills; we help each other to provide the best possible rehabilitation for our patients. Our five physiotherapists and six occupational therapists look forward to welcoming you to their community.

As a physiotherapist in the homecoming team, you are responsible for a safe and secure homecoming of patients from inpatient care. Together with the SVU responsible nurse and the care manager, you will follow the patient's care journey and plan for discharge through careful, person-centered, and patient-safe mapping in Mina planer. You create conditions for the individual to live as independently, participatively, and meaningfully as possible based on a person-centered approach focusing on the healthy. You have an approach where the patient and relatives are co-creators in the individual’s care.

You will collaborate in a team with other professional groups both internally and externally, such as care managers, nurses, occupational therapists, and nursing assistants. The work also involves reporting to other operations in connection with discharge from the hospital and short-term accommodation. Good collaboration creates conditions to provide patients and relatives with security and good service with a focus on rehabilitation.

Competence

Qualifications

• Licensed physiotherapist.
• Experience working as a physiotherapist within municipal operations and/or inpatient care.
• Holds a B driver's license.

Meritorious

• Experience with the SVU process and short-term process.
• Experience in interdisciplinary work.
• Experience working in Mina planer and Lifecare.

You have good service sense and communication skills and are attentive to the individual needs of patients. Furthermore, you have good computer skills and can express yourself well in both spoken and written Swedish. We work in a fast process and therefore require that you have a good ability to adapt to unforeseen changes. Working hours/duration

Permanent full-time employment.

Working hours are scheduled during the day, Monday to Friday.
