District Nurse/Nurse in Home Care

Home Care
The goal of home care is collaboration for good and safe care, and for patients to feel secure, well received, and involved in the design of the services we offer.
Home care is divided into multiprofessional teams with district nurses/nurses, occupational therapists, physiotherapists, and nursing assistants. Home care is divided into three geographical areas: northern and central Gotland, Visby, and southern Gotland.
Medical services are managed by the patient's health center and, in some cases, by the responsible physician at the specialist clinic. We also work on behalf of 1177 and the palliative team.
We are now looking for a colleague for a longer temporary position in the Visby team.
Your Responsibilities
As a district nurse/nurse in home care, you will carry out planned and sometimes urgent interventions within your geographical area of responsibility. The work is free and independent, and you plan your days in consultation with your colleagues. In addition to patient-related work, including assessments, examinations, information, and advice, you also play an important role in collaboration with occupational therapists and physiotherapists in your multiprofessional team, as well as with the base staff in home care and with physicians in primary care. The work also involves close contact with other external care providers and representatives for the patient.
As a district nurse/nurse in home care, you will work daytime with duty every fourth weekend. Some evening shifts may be relevant in the future.
Who Are You?
We are looking for you who are a licensed nurse with experience in the profession. A B driver's license is required.
It is advantageous if you have specialist training. You are confident in making independent decisions and assessments, prioritizing based on the situation, and are used to working closely with others.
In addition to good collaboration skills, you are relationship-oriented, and in your interactions with patients and colleagues, it is important for you to be perceived as empathetic and attentive. You are a team player who sees the whole and the best for the workgroup.

Our areas of operation include social services, home care, elderly care, and care for people with disabilities throughout Gotland.
