Social Welfare Officer for the Homecoming Support Team

We are now looking for a Social Welfare Officer for the Homecoming Support Team. With us, you have the opportunity to start working as early as spring, and we offer individual introduction with mentorship.

We are a total of 28 employees, and the team consists of social welfare officers, nurses, occupational therapists, physiotherapists, and nursing assistants. The team is responsible for the entire customer process from planning in the hospital to the end of the homecoming support period after about three weeks. The purpose is to enable customers in need of continued care and support to return home after hospitalization. Recovery, training, and rehabilitation will take place in the home environment. The team has a close collaboration with Region Skåne.

JOB RESPONSIBILITIES

As a Social Welfare Officer, you will have independent responsibility for processing, follow-up, and evaluation of interventions according to SoL within the care administration. You will collaborate with several teams and report to the unit manager, whom you will be close to. Your tasks include investigating an individual's right to support and assistance based on their needs and circumstances. Primarily, this means investigating, deciding, and following up on interventions according to SoL. Daily work includes care planning, documentation, individual meetings, and collaboration with other staff, relatives, and other external authorities. Documentation is done in Life Care. As a Social Welfare Officer, you are expected to contribute to maintaining high quality in the service with satisfied customers. You contribute to a good working climate and team spirit based on the guiding principles: security, independence, and participation. We have a great working community and a developing profession with a diversity of meetings.

QUALIFICATIONS

We are looking for someone who has a degree from the social work program or other education that the employer assesses as equivalent. We prefer that you have good knowledge of applicable legislation for the work area. Experience in processing according to SoL and/or LSS is an advantage.

Since communication and documentation are part of your work, you need to have good computer skills and good knowledge of Swedish in both spoken and written form. It is an advantage if you have previously worked in Life Care's processing portal. For the position, you need to have a B driver's license for manual transmission.

As a person, you are independent, development-oriented, and capable of taking responsibility. You find it easy to collaborate and communicate in speech and writing. You are clear, structured, and have a positive attitude, as well as being self-assured and having a service-oriented approach.

OTHER

The care administration's activities are aimed at the elderly and individuals who need functional support, as well as those in need of home healthcare. The activities are functionally divided into four areas: support in ordinary housing, support in care and nursing homes, health and medical care, and functional support. The administration also includes the authority unit, the national gymnasium, and the assistive technology center in Eastern Skåne.
