Your Mission

Our main task is to investigate, decide, and follow up on assistance and interventions in accordance with the Social Services Act (SoL) and the Act on Support and Service for Certain Disabled Persons (LSS).

Together, we work to develop our unit and contribute to the vision and community development of Söderhamn.

What Can You Expect?

As a care manager, you are an important part of our team, where you will have the opportunity to work closely with our citizens and build relationships. Your role involves processing and investigating the need for various interventions, such as home care, security alarms, and day activities, as well as making decisions that significantly impact individuals' well-being. You will also follow up on interventions to ensure that the citizens we support receive the help they need.

You will be part of a competent and supportive team with caseworkers who socially support each other in cases. The work is done in teams of two or more employees, which means that collaboration and knowledge exchange are in focus. You will work closely with colleagues, managers, and specialists in authority operations, creating a safe and inspiring work environment.

Focus on Development and Support

To ensure the highest quality and equality in our assessments, we offer regular case supervision. We work based on IBIC (Individual Needs in Center), a needs-oriented and systematic approach that includes structured documentation, allowing us to provide our citizens with the best possible service.

Description of the Workplace

The Care Unit is part of the Authority Department within the Welfare Sector, where we collaborate with the Supply Unit and the Adult Unit. Our unit consists of a dedicated team of care managers, LSS caseworkers, housing coordinators, specialists, and a unit manager.

Your Competence

We are looking for someone with a university degree in social work or another relevant education that the employer deems equivalent. You have good knowledge of authority operations and a solid understanding of applicable legislation.

Experience as a care manager or LSS caseworker is an advantage. It is also beneficial if you have experience with the Lifecare operational system and knowledge of working according to IBIC (Individual Needs in Center).

You have a good ability to express yourself in Swedish, both verbally and in writing. Since travel is part of the job, a valid driver's license (B) is required.
