About the DepartmentThe Primary Care Multimodal Rehabilitation Team for Chronic Pain is located in three towns in Dalarna: Ludvika, Falun, and Mora. The team in Ludvika is undergoing a restart and will work with targeted, coordinated, and integrated interventions based on a clear biopsychosocial perspective. The target group is patients with chronic pain (at least 3 months), where previous interventions in primary care have not been sufficient and where there is a need for a unified team approach.
The focus of the work lies on:
- Physical activity, function, and activity capacity
- Psychological and social consequences of chronic pain
- Support for return to work, studies, or other meaningful life activities
The activity is based on the national care pathway for chronic pain and is an important part of Region Dalarna's development of a more cohesive and person-centered care. We offer:

As an occupational therapist/rehabilitation coordinator in the team, you play a central role in supporting the patient to regain structure, balance, and function in daily life. You work both independently and in close collaboration with the team, where you jointly plan and implement the rehabilitation.
Areas of work for the occupational therapist:
- Occupational therapy assessment
- Activity focus (e.g., ReDO method)
- Interventions regarding structure and strategies (e.g., HaKoll method and the 1177 program Structure Your Daily Life)
- Lifestyle habits (diet, sleep habits, physical activity, etc.)
- Everyday ergonomics
- Hand/orthoses
- Assistive devices
Other responsibilities:
- Coordinate and organize care for patients enrolled in the team. You work both independently and within the team with other professions, both internally and externally.
- Lead rehab meetings, document care plans, and follow them up with the team.
- Participate in multimodal team interventions together with physiotherapists, psychologists, and doctors.
- Collaborate with health centers, specialist care, and other actors as needed.
- Contribute to follow-up, quality work, and registration in quality registers (NRS).
- Work with digital tools such as video visits and support via 1177.

QualificationsRequirements:
- Licensed Occupational Therapist
