About the workplace

We are a small, family-oriented health center located within walking distance of the Upplands Väsby commuter train station. We currently have approximately 5,000 registered patients. Our home care service covers just over 70 patients.

Our home care team consists of two nurses and two nursing assistants. We conduct home visits by car or e-bicycle/bicycle.

Responsibilities

• You are responsible for your own patients together with the nursing assistant in your team, and you conduct patient admissions and discharges.
• Convene SIP (Interprofessional Collaboration) meetings when necessary, and maintain contact with relatives, other units, and the municipality.
• You also make daily home visits.
• Standard tasks within home care.

Qualifications

• We are looking for you who are a licensed nurse or district nurse and are interested in working within home care.
• A few years of experience from primary care/home care is meritorious.
• A valid driver's license (Category B) is required.
• Good knowledge of Swedish, both spoken and written.

As a person, you find it easy to collaborate with others and have good communication skills. You have strong organizational skills and can plan long-term. You are flexible, receptive, and meticulous. You can take initiative and are confident in your profession. You can work independently but also function well in a team.
