Then we need you! We offer you an exciting and developing assignment. At our workplace, we work with commitment, collaboration, and a shared responsibility to develop operations, always with the citizens' best interests in focus. Welcome to the unit for prevention and outpatient care!

Job Responsibilities

The purpose of the SSPF model is to prevent children and young people from developing norm-breaking behaviors, falling into crime, substance abuse, or otherwise risking an unfavorable development, through early collaboration. Through close cooperation between social services, schools, police, and leisure activities, conditions are created for coordinated and individualized support for the young person and their family.

As an SSPF Coordinator, you play a central role in coordinating and leading the SSPF work in Nynäshamn Municipality. You are responsible for ensuring that collaboration between relevant actors functions well and that planned interventions are carried out, followed up, and developed together with the young person, guardians, and collaboration partners.

The assignment includes, among other things:

  • Motivating children, young people, and guardians to receive support and participate in coordinated interventions
  • Receiving and assessing incoming signals about children and young people who may need coordinated support, and initiating SSPF collaboration
  • Planning, leading, and following up on SSPF meetings
  • Conducting mappings, follow-ups, and evaluations of interventions and methods
  • Developing, implementing, and quality-assuring methods, routines, and working methods within SSPF
  • Being responsible for documentation, statistics, and follow-up of the operations.

The role involves close collaboration with both internal and external partners. You collaborate with various social services operations, schools, police, leisure activities, civil society, and the municipality's SIG (Social Intervention Group) coordinator. The assignment covers both individual-level coordination and development work at the group and area level to strengthen preventive work for children and young people in Nynäshamn Municipality.

About the Workplace

The main task of the Social Services Administration is to offer individualized support and services within elderly care, disability services, children and family, adults, and economic assistance. We are present for the residents throughout all stages of life.

You will be part of the unit for prevention and outpatient care within the Social Support Department. The unit offers support to residents of all ages through promotion, prevention, and outpatient interventions. The new Social Services Act strengthens our focus on early interventions, preventive work, and close collaboration. Therefore, we work based on knowledge and start from the individual's needs to strengthen health, participation, and security throughout life.

As an SSPF Coordinator, you will become an important part of the municipality's combined preventive and crime-preventive work for children and young people. SSPF is an established collaboration model where social services, schools, police, and leisure activities collaborate to early identify and provide support to children and young people at risk of norm-breaking behavior, substance abuse, or crime.

With us, you get a varied and developing assignment where you have the opportunity to influence, drive development, and contribute to strengthening preventive work in Nynäshamn Municipality. We value collaboration, trust, and commitment, and believe that the best results are created when we work together to give the municipality's residents the right support at the right time.
