Do you want to make a real difference for people with cognitive diseases and their relatives – from the first contact and throughout the entire support chain? We are now looking for an engaged Dementia Nurse for the Assistance Unit in Upplands-Bro Municipality.

About the position:

As a Dementia Nurse at the Assistance Unit, you will play a central role in developing and ensuring early, coordinated, and needs-adapted interventions for people with dementia. By being placed in the Assistance Unit, you become an important part of the first meeting with the municipality, where you can contribute specialist knowledge at an early stage. You act as a link between healthcare and municipal services, working closely with assistance officers to create security and quality in the support provided to the individual.

In this role, you will among other things:

• Provide advice, support, and guidance to people with dementia and their relatives.
• Participate in home visits in the early stages after diagnosis.
• Contribute knowledge about municipal services, social support, and the role of primary care.
• Collaborate with assistance officers and other professions to plan interventions.
• Serve as a resource for the organization through competence development in cognitive diseases, implementation of new knowledge and methods, and participation in organizational development.
• You are also a resource for the entire elderly care sector and can contribute to various parts of the operation as needed.

About the workplace:

The position belongs to the Assistance Unit within elderly care, which is a strategically important entry point for services and support for people over and under 65 years of age. Here, we work close to the citizen and in collaboration with other functions to meet individual needs. The unit consists of 12 case workers and two employees. You report to the unit manager, while having many opportunities for collaboration outside your direct work group.

We are looking for you who:

• Are a licensed nurse with further education in dementia or geriatrics.
• Have experience working with people with dementia.
• Have good knowledge and experience of working from a pedagogical approach and contribute to knowledge dissemination on a broad front.
• Speak and write Swedish fluently.
• Hold a valid driver's license (Category B).

It is meritorious if you have worked within municipal elderly care and with assistance issues.

As a person, you have a good ability to work both independently and in collaboration with others. You have excellent communication skills and are confident in meetings with both individuals and relatives at different stages of the disease.
