About the position

The Assistance Case Manager Unit in Vilhelmina Municipality is now looking for a full-time assistance case manager for a permanent position; a probationary period may apply.

Your tasks

As an assistance case manager, you work with authority exercises and assistance assessments according to current legislation.

Your main tasks are to:

• Investigate, assess, and make decisions according to the Social Services Act (SoL) and the Act on Support and Service for Certain Functionally Impaired Persons (LSS)
• Follow up and review measures
• Maintain contact with individuals, relatives, and other actors
• Collaborate with executive managers within elderly care and LSS operations
• Cooperate with primary healthcare, the region, and other authorities
• Contribute to the operation's development and quality work

The target group includes both persons under 65 years of age with disabilities and elderly persons over 65 years of age.

Qualifications

• Degree in Social Work or other education that the employer considers equivalent and relevant for the field
• Good ability to express yourself in speech and writing
• Class B driver's license

It is meritorious if you have:

• Experience in assistance case management or similar authority exercises
• Knowledge of current legislation
• Experience in operations within elderly care or LSS
• Experience with the operation system Treserva
