Job Responsibilities
As an intervention case manager, you follow up on interventions approved by investigating social secretaries for children and young people aged 0–20. This includes both outpatient interventions and placements in the form of residential care for children and youth (HVB), supported housing, Swedish National Board of Institutional Care (SiS), and joint placements with the Social Services Act (LSS). You follow the case, ensure that the approved intervention yields results, and actively work to increase protective factors. We strive to work according to the "Signs of Safety" approach.
You will also conduct preliminary assessments regarding the children whose cases you handle. In cases of significant concern, the investigation is passed to an investigator, while you continue to follow up on the intervention. On the unit, there is ongoing work to review all routines and adapt them to the new organization. We look forward to sharing your experiences and receiving your feedback on various matters.
We formalize our cooperation with the police through the BoB Council, collaborate with the field and prevention unit to increase the shared situational awareness across Lidingö, and further develop cooperation with adult services for young people who will need continued support after turning 20. Therefore, here you need to enjoy collaboration and cooperation, with the purpose of offering coordinated support to the residents of Lidingö.
During the year, we actively work on weaving together official authority functions with the collaboration model "Backa barnet" (Support the Child). All to coordinate support for our children and young people with all relevant actors! We are also introducing SIG (Social Intervention Groups) in project form as a needs-assessed intervention. The new SIG coordinator will belong to the Intervention Team and become your new colleague. Our ambition is to bring together the right people for the right issue to achieve the most effect for our children and young people.
Qualifications
We are looking for you who have a degree in Social Work. You have at least 2 years of experience in authority work with children and young people. We would prefer that you are familiar with the working methods available within the Signs of Safety method. It is meritorious if you have training in MI (Motivational Interviewing) and the Ester assessment instrument.
As a person, you are secure and stable. You have good collaborative skills and are good at creating and maintaining relationships. Furthermore, you are accustomed to working in a structured manner and taking great responsibility for your tasks.
Employment type: Permanent position.
Duration: Permanent.
The municipality applies individual salary setting. Start date by agreement.
