WORK ASSIGNMENTS
Do you want to help create a safe, high-quality, and well-functioning home care service? Are you structured, solution-oriented, and at the same time engaged in close care work? Then you might be the Home Care Planner we are looking for.
As a Home Care Planner, you play a central role in the operations, responsible for ensuring efficient, quality-assured, and needs-adapted planning of home care interventions. The assignment aims to create continuity for our users and a sustainable work environment for our staff.
The position is divided between:
- 70% planning work in the planning unit and team
- 30% planned care work in the operations
The working hours are during the day, Monday-Friday, 40 hours per week.
Job Responsibilities
In the role of Home Care Planner, you will:
Planning and Coordination (70%)
- Plan and coordinate daily interventions based on approved decisions and users' needs
- Ensure optimal resource allocation and staffing
- Work actively with scheduling, route planning, and continuity
- Be a support to employees in planning matters
- Collaborate with unit managers, colleagues, and other professions
- Follow up and adjust planning in case of changed needs or conditions
- Contribute to the development of methods and routines within the planning area
Care Work (30%)
- Perform planned care interventions with users
- Contribute to maintaining high quality in direct care work
- Strengthen the link between planning and practical execution
- Act as a role model in work methods, attitude, and value-based work
Your responsibilities as a Home Care Planner:
- Planning is based on the user's needs, safety, and self-determination
- Contribute to high continuity and efficient use of the operation's resources
- Maintain good communication within the team
- Work according to current legislation, guidelines, and routines.
QUALIFICATIONS
We are looking for you who
- Have experience working in healthcare and social care, preferably in home care
- Have good organizational skills and enjoy planning and coordinating
- Are flexible and able to handle changes in operations
- Communicate clearly and collaborate well with colleagues, users, and relatives
- Have a professional and respectful attitude
- Share our values and work with the user's needs, safety, and self-determination in focus
- Experience in planning, scheduling, or staffing is an advantage
- Previous experience with delegation
- Hold a valid driver's license (Category B)
Salary form: Monthly salary.
Employment type: Permanent position.
Duration: Start date: 2026-08-24. Start date according to agreement.
Working hours: Daytime. Working hours during the day, Monday to Friday, 40 hours per week.

If you are not a Swedish citizen, you need to show documents proving your right to work in Sweden.
