We are now looking for a nurse for the functional support team in the city area of Centrum, Örgryte-Härlanda. A working day always begins with joint planning with colleagues, followed by home visits to patients with both somatic and mental health needs in their supported housing. Some days also include rounds, medication deliveries, meetings, contact with other healthcare providers, and collaboration in interdisciplinary teams. You are responsible for planning, implementing, and following up on healthcare interventions in consultation with the patient, nursing staff, and other healthcare providers. The work is primarily carried out through patient visits at home and in day activities. Our patients' needs range from preventive measures to advanced medical care, providing variation, development, and significant personal responsibility. You will primarily work during the day, but some evening and weekend work is included.

About you

We are looking for you who are a registered nurse. Further education in psychiatry or district nursing, as well as previous experience within the target group, is meritorious. We would like you to have experience in the nursing profession, but we also welcome newly graduated candidates. A B-driver's license and driving experience are required as you travel to patients by car.

The role of a nurse requires that you can independently overview, prioritize, and perform your tasks. The meeting with the patient, relatives, and care staff is an important part of the work, so we consider it important that you are sensitive and provide good treatment. To enable us to provide the best possible care, you must also have good collaboration skills, as several people work in teams around the patients. Furthermore, you approach your tasks responsibly and can independently overview, prioritize, perform, and follow up on your duties.
