About the workplace – who we are!
Sweden is undergoing a transition towards "Close Care" (Nära vård). Close Care is a healthcare system based on primary care, where regions and municipalities interact and clearly focus on the individual's needs. This means that care and support within Fagersta Municipality are now developing their mission within the health and medical care group. The municipality's rehabilitation team works in various areas including special housing, short-term units, safe home discharge, home healthcare, LSS (Support and Service for Persons with Certain Functional Impairments), psychiatry, and home visits in ordinary housing.
Your role
Do you want to be part of Fagersta Municipality's journey towards Close Care?
The work involves creating a meaningful everyday life for our residents in Fagersta based on individual needs by working preventively and long-term. The role involves working in teams with various professional disciplines and in close collaboration with healthcare staff and colleagues in the health and medical care group.
You will collaborate closely with your colleagues in the rehabilitation team, which consists of physiotherapists, occupational therapists, and rehabilitation assistants. The work also requires flexibility and collaboration with colleagues in the work group, covering for each other when necessary.
The role also includes being part of the planning for safe home discharge, collaborating with the municipality's coordinator, the region's care coordinator, and the discharge coordinator.
The work as an occupational therapist within the municipality is varied, challenging, and very autonomous, as you distribute the work yourself according to emerging needs. The work includes, among other things, prescribing assistive devices, certificates for housing adaptations, and training in activity performance.
You supervise, educate, and continuously delegate tasks to rehabilitation representatives and healthcare staff to offer good care to our residents in Fagersta.
The work is scheduled during daytime hours. Upon starting your employment, we have developed an introduction program that you will participate in.
Your qualifications and merits
For this position, you must be a licensed occupational therapist and hold a valid driver's license (Category B).
