Sweden is undergoing a transition towards "Close Care" (Nära vård). Close Care is a healthcare system based on primary care, where the region and municipalities interact and clearly focus on the individual's needs. This means that care and support within Fagersta Municipality are now developing their mission within the health and medical care group. The municipality's rehabilitation team works in various areas, including special housing, short-term units, safe home return, home healthcare, LSS (Support and Service for Persons with Certain Functional Impairments), psychiatry, and home visits in ordinary housing.

Your role

Do you want to be part of Fagersta Municipality's journey towards Close Care?

The work involves creating a meaningful everyday life for our residents in Fagersta based on individual needs by working preventively and long-term. The role involves working in teams with various professional disciplines and in close collaboration with healthcare staff and colleagues in the health and medical care group.

You will collaborate closely with your colleagues in the rehabilitation team, which consists of physiotherapists, occupational therapists, and rehabilitation assistants. The work also requires flexibility and collaboration with colleagues in the work group, covering for each other when necessary.

The role also involves participating in the planning of safe home returns, collaborating with the municipality's coordinator, the region's care coordinator, and the discharge coordinator.

Working as a physiotherapist within the municipality is varied, challenging, and very autonomous, as you distribute the work yourself based on emerging needs.

Duties included in the role are, among others:

• Assess needs and determine appropriate treatments/compensatory measures based on a function-preserving approach.
• Fit, prescribe, and follow up on assistive devices.
• Educate healthcare staff in mobility skills and other profession-specific areas.

You mentor, educate, and continuously delegate tasks to rehabilitation coordinators and healthcare staff to offer good care to our residents in Fagersta.

The work is scheduled during daytime hours. Upon starting your employment, we have developed an induction program that you will participate in.

Your qualifications and merits

The position requires that you are a licensed physiotherapist and hold a valid B-class driver's license.
