Nurse for Ordinary Housing Vittsjö/Bjärnum

The assignment includes traditional tasks for a care coordinator nurse within municipal primary care. Supporting and motivating staff, relatives, and other caregivers is part of your responsibilities. The work involves contacts within your own operation as well as with other healthcare providers, inpatient care, primary care, and ambulance services. You will interact with medical technology and patients with complex care needs.
We want your work to be characterized by an ethical approach and carried out in accordance with current legislation and guidelines. As a person, you have a good attitude towards others, work for good results, and take responsibility. You work methodically and with a scientific approach.
Today we use the journal system Procapita, and in 2027 we will switch to Lifecare. The work also includes working with quality registers such as BPSD and Senior Alert.
You will become an important part of the team that leads and develops health and medical care interventions in close collaboration with nursing staff, occupational therapists, and physiotherapists – always with a person-centered approach.

Requirements:
- Registered Nurse (RN)
- Good knowledge of Swedish, both spoken and written
- Good computer skills (documentation is done in Procapita/Lifecare)
- Valid driver's license (Category B)

Working hours are daytime 38.25 hours/week with duty every 5th weekend.

Within the social care administration, we provide care, support, and assistance to people who are elderly, have a disability, or need home healthcare. We also offer health-promoting measures and support to people who care for and support a relative. Our goal is that those who need care and assistance in their daily lives should be able to live an independent life as far as possible.
The Health and Medical Care Department is responsible for interventions within home healthcare and rehabilitation. The interventions are performed by nurses, assistant nurses, rehabilitation assistants, occupational therapists, and physiotherapists, who also delegate to care staff within other areas of operation based on their profession.
