About the Unit

Our units consist of assistance coordinators, senior assistance coordinators, resident coordinators, and administrators. Our target group consists of individuals over 18 years of age who apply for support and assistance that is not covered by the LSS (Act on Support and Service for Persons with Certain Functional Impairments) or does not fall under the target group for processing within social psychiatry. Our target group is constantly growing, which requires our operations to dare to try new working methods to best meet the challenges of the welfare state. At our workplace, we experiment with new methods and continuously evaluate them. You are expected to act according to our value base in all contacts and work proactively.

On our units, the Investigation Unit for Care 1 and 2, we all have different perspectives on things, but we have jointly decided that our work environment should be permeated by acceptance, security, and positivity, where everyone is equally important.

Job Responsibilities

We offer a developmental assignment where you will have the opportunity to contribute to the development of cooperation between Gävle Municipality and Region Gävleborg. The role involves varied work with both operational and developmental elements in a business with a strong focus on collaboration.

The position begins as a one-year project appointment, with a clear plan for continued employment. After the project period ends, the position will transition to a permanent appointment within the Investigation Unit for Care at Gävle Municipality.

We are now looking for you with a degree from the Social Work program or the Social Care program for a developmental role at the Emergency Department within Region Gävleborg, where you will play a key role in reducing admissions and strengthening trust in Gävle Municipality's interventions.

The assignment is a joint development project between Gävle Municipality and Region Gävleborg. The purpose is to contribute to a more sustainable use of healthcare and social care by:

• preventing unnecessary admissions to inpatient care
• strengthening trust in Gävle Municipality's interventions
• enabling the right support to be introduced at an early stage

You are employed by Gävle Municipality with a primary placement within the Investigation Unit for Care, but you work operationally 50% at the Emergency Department within Region Gävleborg and 50% at the Investigation Unit for Care at Gävle Municipality.

The work involves assessing the need for interventions according to the Social Services Act (SoL) and making assistance decisions in close connection with patient flow. You collaborate with healthcare staff, patients, and relatives, and contribute to making Gävle Municipality's interventions visible as an alternative to inpatient care.

The assignment also includes a clear developmental perspective where you participate in developing and testing new working methods. The role is partly shaped within the framework of the assignment.

Qualifications

We are looking for you who:

• have a degree in Social Work or from the Social Care program
• have experience in authority practice, preferably within elderly care or a similar area
• have good knowledge of the Social Services Act
• have the ability to make independent decisions in various everyday situations

It is an asset if you have experience of cooperation with healthcare.
