Responsibilities

As a Care Coordinator, you work to secure the individual's right to security, participation, and influence when care, support, and assistance are planned and implemented. Initially, you serve as the patient's fixed point of contact, guiding them and providing security throughout the process. When necessary, you facilitate or establish appropriate contacts with other professionals within the region or municipality. You play a vital role in our mission to ensure compliance with the law regarding safe and secure admission and discharge from inpatient care.

In the role of Care Coordinator, you act as the primary care link in the collaboration between inpatient care, primary care, and municipalities. One of your main tasks is to work according to the ViSam model. You are based at Lindesberg Health Center but collaborate with other region-run health centers in the northern part of the county. You convene Coordinated Individual Planning (SIP) meetings, a tool where you, together with the patient and relatives, create a plan for a safe and secure daily life. You will also take a leading role in the work towards "Close Care" (Nära vård), which means shifting focus from the organization to the person and the relationship. Together with colleagues, you actively work to develop collaboration with the municipality, where an enhanced collaboration offers opportunities for more resource-efficient and person-centered work around the patient.

Qualifications

Experience and education within health and medical care are required; we prefer that you are a registered nurse. Experience working in similar coordinating assignments is an asset, as is experience working according to the ViSam model. Experience with information transfer systems between healthcare providers, such as Lifecare, is also considered an asset. You must have good skills in using digital technology.
