Your mission

The work as a nurse in the municipality means that you work with and have care responsibility in home healthcare for individuals living in regular housing, special housing, short-term units for the elderly, group homes LSS, and social psychiatry housing. As a nurse with us, you will be part of a health and medical care team with physiotherapists, occupational therapists, and rehabilitation assistants/nursing assistants.

In consultation with the individual, assessments of care interventions are made. You will perform and follow up on medical orders such as medication management, wound care, etc., and investigate and assess risks such as blood pressure, weight, nutritional intake, and skin, and follow up on these.

You are responsible for planning and conducting SIPs, care planning, and team meetings with the patient in focus with the interdisciplinary team.

You will educate and guide patients, staff, and relatives. You will work closely with unit managers, nursing staff, and caseworkers.

We work every fourth weekend, day/evening according to the schedule.

Your qualifications

You are a licensed nurse. We are looking for someone who finds it stimulating to work both independently and in a team. You have a holistic view of the patient with innovative and reflective approaches. A person who has a good ability to collaborate with and guide colleagues, nursing staff, as well as patients and relatives.

You are comfortable with IT-based journal systems and have a good ability to document. You can express yourself well in spoken and written Swedish. A B driver's license is a requirement.

You enjoy working with people and helping those who need support and assistance to live as independently as possible. You like challenges, dare to take your own initiatives, are willing to learn new things, and are open to trying new things and finding new solutions.

Experience in the profession is a plus, but most importantly is you as a person, and that you have a genuine interest in the work and a desire to develop.

Other

The position is a permanent full-time position, with start date according to agreement. Selection and interviews may occur on an ongoing basis.
