The Social Administration's task is to support people who, due to disabilities, economic or social issues, need various forms of support and assistance.

We are looking for a nursing assistant or nurse for the role of care coordinator.

Are you a nursing assistant or nurse who wants to work with the care of the elderly? Do you want to be a key person in creating safe and cohesive care in the patient's home? We are now looking for a dedicated care coordinator who wants to help develop and coordinate the work within home healthcare.

Description and Responsibilities

You will be part of a health and medical care team together with physiotherapists, occupational therapists, rehabilitation assistants, nursing assistants, and nurses. You will closely collaborate with unit managers, care staff, and primary care doctors.

As a care coordinator, you will participate in SIP, care planning, and patient admissions to home healthcare. You will be responsible for establishing and documenting health plans, collaborating with health centers, and meeting with patients during their admission to home healthcare.

Internal collaboration occurs with nurses, occupational therapists, physiotherapists, and case managers to ensure person-centered care where the patient feels safe and knows who is doing what.

If space allows, you will assist in the clinical operations.

Qualifications:

• You are a trained nursing assistant or a licensed nurse.
• You have experience in healthcare and social care, preferably home healthcare.
• You have good communication and collaboration skills as well as a positive attitude.
• You are attentive, flexible, and able to share knowledge and information in a pedagogical manner.
• You must have good knowledge of the Swedish language and be proficient in both spoken and written form.
• Good computer skills are essential to perform the duties as we work in various IT systems.
• A valid driver's license (B) is required.
