Linden Health Center has approximately 2600 registered patients. We provide standard primary care with family doctor services, rehabilitation, psychosocial therapy, and child healthcare. Our team consists of doctors, nurses, district nurses, diabetes nurses, asthma/COPD nurses, physiotherapists, occupational therapists, psychosocial therapists, and medical secretaries. We also have a close collaboration with Söderhamn Health Center.

JOB RESPONSIBILITIES

As a district nurse/nurse with us, you will collaborate closely with other professionals at the health center, contributing together to ensure our patients receive safe and high-quality care. Your expertise is a vital prerequisite for our shared mission to support, treat, and guide people towards a healthier life. The role includes some duties at the family doctor emergency service in Söderhamn.

As a district nurse/nurse, you will work with:

• healthcare for both children and adults
• telephone advice
• clinic work with scheduled and urgent visits
• preventive healthcare
• home visits and care planning.

QUALIFICATIONS

We are looking for a flexible and committed person who thrives in close collaboration with all health center professionals in a varied workday. You are interested in development and improvement work, confident in your professional role, and have the ability to meet people in a professional and empathetic manner.

In the role of district nurse/nurse, you should:

• be a licensed nurse or have district nurse training
• hold a valid B driver's license.

For this position, it is advantageous if you have experience working in primary care, experience in elderly team work, Lifecare, and the Cosmic journal system.
