Description

The role of Planner requires a stable, stress-resistant person with good collaborative skills.

You will be one of six planners who work daily to schedule minute-driven interventions for our clients in ordinary housing using planning tools in Life Care Planning. The home care planner works with all ten of our units, distributed across areas in the center, outskirts, and night patrol.

You will have close contact with employees, unit managers, the staffing unit, licensed personnel, and assistance assessors. We have daily close contact through phone and our internal communication channels.

Responsibilities

As a planner, you will be responsible for:

  • Daily planning of clients' approved interventions based on personnel continuity and competence
  • Receiving and confirming new decisions from assistance assessors
  • Receiving and scheduling health and medical services interventions from licensed personnel
  • Observing discharge planning for clients via cosmic link, Region Värmland's emergency journal system
  • Reporting staffing and planned time to the economist
  • Supporting unit managers and schedulers in creating graphs for staffing needs
  • Supporting staffing planners in filling in during short-term absences

Responsibilities may change based on the needs of the organization.

Qualifications

To be considered for the position, you should have:

  • Relevant education or experience in the field deemed equivalent
  • Experience in administrative work in personnel systems and working with planning tools like Life Care Planning or equivalent, as well as experience with journal systems
  • Several years of experience using a computer as a working tool and good digital skills
  • Experience in operational work with planning tools LMO, Life Care Mobile Care, or similar within home care in ordinary housing
  • Relevant experience regarding collective agreements and working time regulations
  • Good ability to communicate in Swedish, both spoken and written
