Rehab Väst is a unit within primary care in western Östergötland that offers high-quality rehabilitation to patients in the western part of the region. Our mission is to provide Östgötar with safe, quality-assured, and equitable rehabilitation regardless of where in the region they live. We currently operate at the hospital in Motala, in Mjölby, and at several health centers in the western part of the region. Rehab Väst has over 80 employees consisting of physiotherapists, occupational therapists, social workers, speech therapists, and administrators. We offer rehabilitation within inpatient care, outpatient care, and primary care. Within primary care in the west, there is close collaboration between the units, which creates good conditions for development, participation, and cooperation with other care providers. We also have close collaboration with other rehabilitation organizations in the region. Our operations are characterized by commitment and the desire to develop and improve processes to create added value for the patient. Within Rehab Väst, we support and help each other and are knowledgeable in our professions and missions. We actively work to use and develop digital solutions to meet the needs of citizens and the organization.

Job Description

We are now looking for an occupational therapist for a very rewarding and developing job at LAH and the emergency department at Rehab Väst.

In the LAH assignment, you work with the primary care clinic in the west. The work at LAH mainly involves palliative care and home visits. During home visits, you often work alone in the patient's home. You will have close collaboration with the rest of the team at LAH. You will be placed in both Mjölby and Motala.

At the emergency department, you work together with the team there and conduct activity assessments and interventions to ensure and facilitate discharges from the emergency department. There is significant collaboration with other actors, and the work involves a lot of care coordination.

The role of the occupational therapist involves assessing and rehabilitating patients. Your work with us includes a lot of collaboration and coordination with other rehab colleagues, the team at LAH, municipalities, and other care neighbors. We document in the Cosmic journal system.

Work Group

About You

We are looking for a licensed occupational therapist. Experience in palliative care, inpatient work, and care coordination is an advantage. The job requires you to have good language skills in Swedish and English. A B driver's license is a requirement.
