We are a pleasant and well-functioning health care unit with approximately 11,300 registered patients and around 40 employees. Our team consists of engaged and competent staff from all professional backgrounds, working with a wide range of healthcare services. We have specialized clinics for diabetes, asthma/COPD, and heart failure.

We work in interdisciplinary teams and have a well-established emergency team with doctors, nurses, and nursing assistants, and we are also in the process of establishing interdisciplinary teams for other patient flows. If you have education and experience in heart failure clinics, there is now an opportunity to start working and taking responsibility for this area with us.

JOB RESPONSIBILITIES

We now welcome you who are a registered nurse or have further education as a district nurse.

If you have experience in primary care and want to work in a workplace where we strive to be the best choice for our citizens, and if you want to share your knowledge and experience to make us even better, then the role of nurse/district nurse with us may be right for you.

In your role as a nurse/district nurse, you will assess various health conditions and refer patients to the appropriate level of care. Telephone triage is a shared responsibility for all nurses and district nurses in the organization, and this work is combined with tasks in the emergency team, in interdisciplinary teams, as well as with reception work.

QUALIFICATIONS

We welcome you as a licensed nurse or district nurse with the authority to practice within Swedish healthcare. Language skills in Swedish at least equivalent to level C1 according to the Council of Europe's scale are required. Furthermore, we would like you to have experience working in a health center and experience with the PMO journal system. Experience in managing heart failure clinics is an advantage.

You have a good ability to see and meet the patient and their relatives. In your work, you are structured and have an easy time prioritizing among your tasks.
