District Nurse/Nurse in Home Care
Do you want to work as a District Nurse/Nurse in home care? We are looking for someone with experience as a nurse, preferably within municipal care and nursing. If you have a specialist education as a district nurse, geriatric nurse, or other relevant specialization, it is an advantage.

The Health and Medical Care Unit (HMSV) is a branch within the social services with the purpose of promoting primary care as the hub of health and medical care for the municipality's citizens. HMSV works to ensure that the municipal care level functions closely with citizens and contributes to health and security, as well as promoting and developing person-centered working methods. As we face significant challenges in future care, preventive care and promoting independence for our patients is an important mission and goal.
Organizationally, we are divided into two units where HSE1 (Health and Medical Care Unit) includes home care, LSS, home rehabilitation, and assistive technology. HSE2 includes care and nursing homes.
There is a wide range of expertise within the staff group, which also has a strong desire to develop and improve health and medical care for the citizens of Kumla. Behind you stands an engaged and present unit manager with a nursing background and extensive experience in municipal care and nursing. We work to ensure that tasks and the work environment are sustainable, where recovery and development are important. As a unit manager, I place great importance on mutual trust and being unpretentious in our relationships.
HSE1 consists of 9 occupational therapists, 2 assistive technology technicians, 17 nurses, and one unit manager. We are based at the Home Care House, the Old Shoe Factory in Kumla, where there are also 4 home care groups, unit managers, and administrative staff.

JOB RESPONSIBILITIES
You will belong to home care and perform assignments for patients in their own homes. Here you will work in a team together with nurses, occupational therapists, care staff, and the unit manager.
Your main tasks will be to plan, implement, and evaluate health and medical care interventions based on each individual's needs, make medical assessments, guide care staff, and maintain contact with and provide support to relatives. You will also be responsible for medication management, documentation, and participate in various collaboration meetings within the team.
QUALIFICATIONS
We are looking for you who are a licensed nurse, district nurse, geriatric nurse, or have another relevant specialist education and experience. You have good collaboration skills and contribute to a positive, helpful, and developing work climate. We expect you to be able to work in a structured, independent, and flexible manner based on a professional approach with the ability to see opportunities. You are used to using technology and digital tools in your work and express yourself well in speech and writing. A valid driver's license (B) is required.
Experience in home care and familiarity with documentation in Procapita, NPÖ, Pascal, MCSS Appva (digital signing), and Lifecare is an advantage.
