Description

We are looking for a nurse for the Municipal Primary Care unit to work within the special housing team, which also includes short-term care. You will be part of a team with nurses as well as a cross-professional team with occupational therapists, physiotherapists, unit managers, support staff, and other professionals surrounding the patient, focusing on safe and person-centered care. You have great opportunities to plan and structure your workday to suit you, your patients, and your colleagues. You will be the responsible nurse for one of our central housing facilities in Sunne, as well as a resource for other facilities for a certain period. Our facilities are within walking and cycling distance or accessible by car. You will work daytime on weekdays and approximately every fifth weekend, with the possibility of flexible working hours. We offer individually tailored introductions, mentorship, and collegial guidance.

Responsibilities

Your main responsibilities include coordinating and ensuring that specific nursing interventions are planned, implemented, and evaluated, as well as educating, guiding, and delegating to support staff. You will perform medical and specific nursing interventions based on patients' needs. The work involves planning and following up on nursing interventions either independently or together with colleagues. The operational systems that support us in our work include digital signing lists and the Life Care HSL journal system.

Qualifications

We are looking for a licensed nurse with a commitment and interest in municipal primary care focusing on the elderly and dementia. It is an advantage if you have worked with similar tasks before. You are welcome to have specialized training in district nursing, elderly care, or psychiatry. To succeed in this position, you work independently and take significant personal responsibility for your profession as well as for planning and following up on healthcare.

You highly value teamwork and have a strong interest in working with vulnerable elderly, dementia, and palliative care. As a person, you are stable and flexible, have a strong sense of responsibility, and can work both independently and in a team. You are driven by challenges and want to develop both within your profession and in municipal primary care. We place great importance on personal suitability.

Conditions

Swedish license as a nurse. A B driver's license (manual) is required, as well as the ability to cycle.
