JOB RESPONSIBILITIES

In the role of a nurse in municipal primary care, you will be responsible for health and medical care interventions in patients' homes. Your work includes medical assessments, risk prevention measures, medication management, and providing support and guidance to nursing staff. You will plan and follow up on care interventions based on each patient's individual needs and situation.

You will collaborate closely with other professionals within and outside the municipality, such as nursing assistants, doctors, and rehabilitation staff. The position is suitable for you who thrive in independent work while valuing collaboration and have the ability to coordinate and keep care interventions together. We work from a person-centered approach and a holistic view, where each patient interaction is unique and significant.

QUALIFICATIONS

We are looking for you who are a licensed nurse, preferably with specialist training in district nursing, psychiatry, or elderly care. You have good experience working with IT-based medical record systems and express yourself well in both spoken and written Swedish.

Experience from care and nursing is an advantage, but other relevant work experience is also valued. A B driver's license is a requirement as the work often involves home visits to patients.

You have the ability to remain calm even under high workload and work with a professional and positive attitude towards both patients and colleagues. Great emphasis will be placed on personal suitability.
