About the position

In Home Care, you will have an independent job where you can help develop the operations. The work is carried out both independently and in close collaboration with other nurses, rehabilitation staff, and care personnel to meet the patients' care needs. You will guide the nursing staff, and the work is done in teams together with home care personnel. Working hours are daytime, Monday to Friday, with some weekend shifts every eighth week. The position is based in Norrtälje.

Your tasks include:

• Basic home care such as wound dressings, catheter care, and medication management
• Establishing care plans
• Assessing patients' needs and follow-ups
• Quality registers
• Delegations
• Collaboration with colleagues in customer choice and other care institutions
• Guiding nursing staff
• Responsible for quality registers, delegations, and care for about 30 patients

About you

For this position, you need to:

• Be a licensed nurse, district nurse, or nurse with further education in elderly care
• Hold a B driver's license and have driving experience
• Have very good knowledge of Swedish, both spoken and written

Preferred qualifications

• Prescribing rights and experience in the profession
• Further education as a District Nurse or in Elderly Care

As a person, you are driven by your medical knowledge while your empathetic ability helps you meet patients and colleagues in a good way. You are self-sufficient and can plan your time according to the patients' needs. The role includes the ability to reprioritize when necessary, which is why it is important that you are confident in making quick decisions. Collaboration skills are also key, as you will work closely with many different professional groups.

About us

Basic home care is part of the customer choice Tiohundra, together with home rehabilitation and home care in Norrtälje. We have a close collaboration with the health centers where our patients are registered, and we are also responsible for housing within LSS and social psychiatry. Together with the care staff in home care, we have regular joint breakfasts and a close collaboration.
