JOB DESCRIPTION

We are looking for a responsible nurse/community nurse to join our HSL team! With us, you will be welcomed by friendly colleagues with varying lengths of experience in municipal care and support, where we work to support and help each other. Together, we form a good team consisting of permanent employees. Evening/night, day, and resource nurses, cognitive support teams, occupational therapists, physiotherapists, rehab assistants, business developers/system managers, MAS, and operations managers.

Important for us in our team is good cooperation, balance between demands and conditions, patient focus, and to laugh and have fun together at work. We are each other's work environment, which is a prioritized area for the operations manager. Leadership based on trust and confidence, with the well-being of employees in focus.

The work is in one of our two home care areas in Bräcke. The job involves standard nursing tasks with the individual's needs at the center, where you assess, plan, implement, and lead treatment for patients together with other professionals in municipal home care. The municipality has a good collaboration with Storsjögläntan, where the team together performs palliative interventions in end-of-life care. A flexible and varied job towards ordinary housing where you are responsible for a specific area, but cooperation is also important between areas when needed. We work in teams together with unit managers, care staff, rehab staff, assistance officers, and nurses. Close collaboration with doctors and other professionals at the health center.

One of many important roles in the job is to mentor, provide support, educate care staff and relatives, and participate in care planning between the municipality and the region. Team meetings and care team meetings are forums in the operations where collaboration takes place together with others in the team.

QUALIFICATIONS

We are looking for you who are a licensed nurse; further education in the profession is an advantage. The job requires being attentive to individual needs, flexible, responsible, and cooperative. Ability to lead staff, provide them support and advice in matters related to the profession. Experience in the profession is a must as you plan and carry out your work independently.

It is important to have a positive attitude and a good approach towards both colleagues and patients and relatives. Great importance will be placed on personal suitability.

Requirement for a B driver's license.

Good commuting opportunities with the possibility to work on the train.

Express yourself well both verbally and in writing in the Swedish language.
