Healthcare Assistant for Mobile Care Team

Job Responsibilities

As a healthcare assistant with us, you will perform specialized care tasks in patients' homes. You will visit patients independently and will follow up on their well-being after discharge from inpatient care.

The work also includes planning, coordination, and patient visits. You will collaborate with nurses and doctors during patient visits and participate in digital rounds together with municipalities and healthcare centers. During evening and weekend shifts, you will conduct patient visits together with a nurse.

About the Workplace

The Home Care Team and AH Team have formed their own unit within the Medical Clinic in Köping/Fagersta. The teams consist of about 10 employees, with a few rotating between the teams and the care department. It is a close-knit team that works together and supports each other across teams.

The Home Care Team in Köping provides specialized treatment in patients' homes and is a planned operation with doctors. We are here for patients who cannot reach healthcare, and our mission is to create security at home and avoid hospital admissions. The catchment area includes Köping, Arboga, Kungsör, and Hallstahammar municipalities.

The AH Team (Advanced Home Healthcare) provides advanced palliative care at home. The work includes symptom control, teamwork, relationship/communication, and support for relatives to alleviate physical, psychological, social, and existential suffering. This involves a strong focus on mutual interpersonal relationships, primarily with the patient but also with their relatives. We are here for patients in Köping, Arboga, and Kungsör.

Qualifications

We are looking for you who are a healthcare assistant with a couple of years of experience in specialized care.

A valid driver's license (B) and driving experience are required. Education in palliative care or intensive care is a merit. As you will need to communicate verbally and in writing with patients, relatives, and colleagues, good knowledge of the Swedish language is required.

As a person, you are calm and secure in your professional role. You work in a structured manner and can plan both long-term and short-term, as well as prioritize among your tasks. You are flexible and can easily adapt to changing circumstances. As collaboration is an important part of the work, we see that you enjoy and have a good ability to work with others. Furthermore, you are empathetic and responsive in your interactions with both patients, relatives, and colleagues.

Employment Conditions

Permanent employment, full-time. Work will be conducted during days, evenings, and weekends.

Start date according to agreement.
