Occupational Therapist at Hedesunda Färnebo Health Center

Hedesunda Färnebo Health Center is located 37 km south of Gävle. The catchment area includes Hedesunda and large parts of Österfärnebo and Gysinge. The population base in the area is approximately 3,900 with 3,350 actively listed. Hedesunda Färnebo Health Center is part of the primary care area of Gävle, which consists of eight publicly operated health centers. We provide primary care based on the health choice assignment with medical consultations, respiratory clinics, home visits, child health care, district nursing, psychosocial teams, occupational therapy, and physiotherapy. JOB RESPONSIBILITIES

Are you an occupational therapist who wants to work with us to enhance our patients' opportunities for activity and health? The position is offered at 100 percent. The need at Hedesunda Färnebo Health Center is 50 percent on-site, and the remaining 50 percent involves being part of the team in the primary care area of Gävle.

As an occupational therapist with us, you will work with:

• assessment of activity ability in consultation with the patient and the need for rehabilitation measures
• assessments, prescribing aids, trials, and follow-ups according to the prescribing process
• memory assessments as a complement to the district nurse's and doctor's assessments/investigations
• formulating statements in cases concerning housing adaptations and applications for personal assistants
• monitoring aid costs and rental responsibilities
• patient education and counseling
• individual treatment largely in collaboration with other professionals
• assessments and occupational therapy measures in the patient's home.

QUALIFICATIONS

We are looking for a flexible and committed colleague who enjoys being part of the development at the health center and within primary care. You are responsible and solution-focused with good collaborative skills as we have a multidisciplinary collaboration.

In the role of occupational therapist, you should:

• be a licensed occupational therapist
• hold a valid B driver's license.

For this position, it is advantageous if you have experience working in primary care and/or home healthcare.
