Your mission

You will assess the individual's ability to perform activities, bodily functions, and environmental factors.

By applying measures such as activity training, provision of assistive devices, and environmental adjustments, you will support and promote the individual's ability to be active, involved, and independent. Furthermore, you will guide and educate nursing staff and collaborate with other team members, including through care planning and team meetings, to develop person-centered health plans.

About the workplace

We provide rehabilitation for short-term care, care homes, and home healthcare in Söderhamn municipality. The unit consists of seven physiotherapists, eight occupational therapists, three rehabilitation assistants, six nursing assistants in Safe Homecoming and IHR (intensive home rehabilitation), and one unit manager. We are co-located, which gives us opportunities to develop together. We work patient-centered in teams together with nurses, nursing staff, unit managers, and social workers, as well as doctors when needed.

We are looking for someone who

Is a licensed occupational therapist. Experience in home healthcare is an advantage. A driver's license is required for the position.

Your role places high demands on independence and responsibility as you will be responsible for planning and organizing your work. You need to be able to take initiative, prioritize, and make decisions within your professional area of responsibility. Since we work in teams, it is important that you have good collaboration skills and that you are solution-focused with the patient's involvement in mind. It is also important to have empathy and a good attitude in all situations. You should be adaptable to changing circumstances, able to see the bigger picture, and consider the broader perspective. As a person, you are positive and motivated to be part of the continued development of the organization.
