The Tollarp Health Center with its branch in Degeberga is located, as the name suggests, in two places, both in Tollarp and Degeberga. We are beautifully situated rural health centers with approximately 8,800 registered patients at the gateway to Österlen. The Tollarp Health Center acquired the Degeberga branch last year and is in the process of developing opportunities for patients seeking care at both locations. This allows staff to work at both centers to meet the needs of our registered patients in both areas. In Tollarp, the health center is located close to the E22 and bus connections to Kristianstad and Malmö, while the branch in Degeberga is located in the village with good bus connections to both Kristianstad and Ystad/Simrishamn.

The staffing at the Tollarp Health Center branch in Degeberga is interdisciplinary with a rich exchange of experiences to meet patients' needs. We offer, among other things, medical consultations, physiotherapy, social work, dietetics, psychology, and occupational therapy.

JOB RESPONSIBILITIES

We now have the opportunity to welcome a social worker for a temporary position of about one year.

At the primary care level, we work with patients experiencing mild to moderate mental health issues. The work consists of assessment, counseling, advice, and rehabilitation of patients. We work with support conversations, therapies, including internet-based CBT. We act as a knowledge support for the patient in their contact with the municipality and other collaborating partners. The work is carried out independently, but you will have close contact with your colleagues. We work as a team with joint assessment rounds, which means that the workgroup collectively assesses and plans for non-urgent patient visits, contributing to high patient safety, and we help prioritize based on the individual patient's needs. Furthermore, we expect you to be involved in improvement work and organizational development.

QUALIFICATIONS

We welcome you with a recognized degree in social work and good knowledge of the Swedish language in both spoken and written form. You may have training in Cognitive Behavioral Therapy (CBT) equivalent to level 1 or other equivalent advanced training. It is an advantage for the position if you are a licensed health and medical care social worker, and in that case, it is a requirement to have language skills in Swedish corresponding to at least level C1, according to the Council of Europe's level scale. We also see it as an advantage to have experience from primary care and/or psychiatric outpatient care. Previous experience in the role of rehabilitation coordinator is also a plus.

As a person, you have a genuine interest in people and work with the patient in focus, while also working towards the goals of the organization. Additionally, you are confident in your professional role, positive, and contribute to a good work environment. Furthermore, you have good collaboration skills and are flexible regarding the varying tasks that may arise during a day at a health center. We place great importance on personal suitability.
