We are now looking for a care coordinator for meaningful work. We are an outpatient clinic at a specialist level that works together for secure care processes where the child's needs are in focus. We offer you a developing operation where you will work with care coordination in collaboration for children and young people in our catchment area of Gällivare, Jokkmokk, Kiruna, and Pajala.
We are looking for
We are looking for someone who is a trained social worker, licensed nurse, or has another relevant higher education. You have good knowledge of Swedish corresponding to C1 level. It is an advantage if you have further education in psychiatry, experience working with children and young people, and knowledge of care coordination and collaboration between different care and community actors. Experience in development and improvement work, as well as knowledge of the Sámi, Finnish, and Meänkieli languages, is also a plus.

You communicate clearly and contribute to a positive and solution-focused work environment.
This is what you will work with
As a care coordinator within Child and Adolescent Psychiatry, you play a key role in creating safety and continuity for children, young people, and their families. Through collaboration with schools, social services, and other actors, you work to coordinate efforts and ensure that each child receives the support they need, including through Norrbus collaboration. In your role, you combine structure and empathy to contribute to effective care processes where the child's needs are always at the center. In addition to your coordinating function, you also work within your core profession, which may involve reception work such as telephone counseling, medication follow-up, group treatments, or individual patient meetings and treatments. Information about the position
We offer you a permanent full-time position. Working hours are scheduled during the day from 8 AM to 5 PM. We apply a background check before making a hiring decision according to the provisions of the Act (2013:852) on background checks of persons who will work with children. Selection and interviews are ongoing, and the position may be filled before the application deadline.
Region Norrbotten applies individual salary setting according to collective agreements.
