JOB RESPONSIBILITIES

In the reception team, you will work in fast processes, where your ability to build trust is crucial. The ability to make decisions on the spot requires your independence and ability to collaborate with other professionals based on the patient's needs.

The reception team is a service that works with the discharge process for patients after hospital stays and short-term stays for a safe return home.

We work cross-professionally. The team includes an occupational therapist, physiotherapist, nurse, nursing assistant, and unit manager. The work also involves collaboration with case managers. The goal is to provide the patient with the opportunity for increased independence through daily training and rehabilitation in their home environment for a period of about 2-3 weeks.

The team collaborates closely with Region Skåne in planning for the discharge. This takes place in the shared IT system Mina planer, where all planning for collaboration during discharge occurs. For the nurse, the work largely involves collaboration with the patient, inpatient care, primary care, and relatives.

Traditional tasks for a nursing responsible nurse within municipal health and medical care are also included. Supervision and delegation of nursing staff are part of the job. The focus is on teamwork, creating opportunities for our patients to feel safe and involved in the planning.

You will also work alongside the nurse responsible for our short-term accommodation.

QUALIFICATIONS

We are looking for a registered nurse with good knowledge of municipal health and medical care. As the nursing responsible nurse, you are part of maintaining patient safety at Landskrona Omsorg. We prefer candidates with experience in the field, and further education in geriatrics or similar is an advantage.

Since communication and documentation are part of the job, you need good computer skills and proficiency in Swedish, both spoken and written. We also prefer candidates with experience in documenting in medical record systems, as well as experience with the systems Mina planer, NPÖ, and Pascal. A valid B driver's license is required for the position.
