Housing Support Worker for the Elderly in Regular Housing

The housing support for the elderly within Regular Housing is a workgroup that started at the beginning of the year. In the role of housing support worker, you will be a support person for a number of individuals who have been granted assistance by the authorities. The role also includes contact with care managers and other stakeholders. We actively work with documentation in our operational system Combine, where we journal and write implementation plans. The role of the housing support worker also includes being a fixed care contact and having assigned areas of responsibility that ensure the operation functions.

Housing support is a social and developmental intervention of a pedagogical nature aimed at helping individuals manage better on their own. The support should be tailored to the individual's needs and opportunities to develop a functioning daily life. The intervention is based on needs and abilities and can be supportive or compensatory, where the individual's own abilities are utilized. The practical support should be motivating, energy-saving, and “initiating.” The support can serve as a learning aid, where the housing support initially participates, with the goal of leading to greater independence for the individual in the future.

The working hours are scheduled and take place during the day but may, if necessary, occur on weekends. The work is based in an office location but is primarily conducted in the individual's home. However, the intervention is not limited to the individual's home but can also include activities outside the home. The support relies on your ability to coordinate, motivate, inspire, and guide individuals in the right direction, as well as to practically initiate daily chores together. The fundamental support lies in structuring and motivating, but also compensating, in the areas where the individual has needs.

Your Mission

Based on goals set by the authorities, you will work with the client to find good routines in daily life for increased well-being and security, leading to greater independence for the client.
The main goal of the mission is to support the client in the way they need to maintain their apartment.
Motivate and work solution-focused as a support that leads forward and provides the best possible effect for the individual.
Present and introduce any aids that can facilitate daily life.
Guide and support the client in housing searches and contact with various authorities and professionals, the region, or other actors within and outside the municipality.
Help the client break isolation and create good relationships.
Help create a meaningful daily life for the client.

Who are you?

You should have a professional approach and always focus on the client and their needs. You enjoy working both independently and in a team, are stress-resistant, flexible, and persistent.

Requirements
- Upper secondary education in healthcare, behavioral science, social pedagogy, or other equivalent education.
- Experience working with mental health issues, addiction, and social vulnerability.
- Very good knowledge of the Swedish language in writing as well as in speech.
- B driver's license

Meritorious:
- Knowledge of languages other than Swedish and English.
- Education in healthcare and social care with a specialization in psychiatry.
- Training and/or experience in low-arousal approaches and pedagogy.
- Further education in psychiatry.
- Training and/or experience regarding NPF and other disabilities.
- Training and/or experience in MI, motivational interviewing.
- Experience working with individuals with long-term mental disabilities.
- Experience in working with addiction issues and addiction theory.
- Experience in care and support coordination.
- Experience working with neuropsychiatry.
