Description

We are looking for an engaged and competent District Nurse who wants to help shape care for individuals with suspected cognitive impairment in municipal primary care in our safe and nature-rich Dalsland municipality! Here, you will work in a team with strong development potential, good cooperation, and a positive outlook on the future, where every employee is significant.

About the Role

As a District Nurse with us, you will have a varied, independent, and meaningful job close to the residents' daily lives. You will become a central part of the municipality's home care service and be responsible for planning, executing, and following up on qualified health and medical care interventions in the home, in care housing, and in other municipal activities when dementia disease is suspected or confirmed.

The work includes, among other things:

• Assessment, investigation, planning, and follow-up of the patient's care needs
• Medication management and medical treatments
• Coordination around the patient together with relatives, care neighbors, and the team
• Support for nursing staff through supervision and competence development
• Documentation and follow-up according to current guidelines and legislation
