We are facing a change where the social services sector and the care and elderly care sector are merging into a new joint sector, the welfare sector. The purpose is to create better conditions for meeting future needs and challenges through strengthened collaboration, more secure competence supply, and more consolidated support for residents.

The work with the new organization is ongoing and may involve changes in department and unit divisions in the coming period.

RESPONSIBILITIES

To meet the requirements of the Social Services Act and the needs we see in operations, we are now expanding our team and looking for another SIP Coordinator.

Today, two employees work on coordinating SIP meetings for children where social services is the client. Now we take the next step and strengthen the work both operationally and strategically.

As a SIP Coordinator, you will become a central part of the municipality's work with coordinated individual plans (SIP). Together with two colleagues, you form a specialist and development resource that supports operations in planning, leading, and following up on SIP processes with high quality.

The assignment involves that you:

• plan and lead SIP meetings based on the best interests of the child and a holistic view of the individual's needs
• coordinate measures between different actors such as schools, social services, and healthcare
• ensure structure, participation, and clarity in the meeting process

An important part of the assignment is also to further develop the municipality's SIP work. This means that you:

• contribute to a more systematic and equitable way of working
• develop methods for follow-up and analysis of SIP measures
• follow up on results with a focus on the difference the measures make for the child

You will work closely with operations within children and young people, with the possibility to eventually develop the assignment to include adults in need of coordinated measures. The role suits you who enjoy working both close to the individual and with development at the system level.

QUALIFICATIONS

We are looking for you who have a degree in social work or equivalent higher education that the employer considers equivalent.

You are a person who

• has experience in social work
• has experience working with SIP
• has the ability to facilitate meetings
• has an interest in developing SIP work, conducting training and information

It is meritorious if you have experience from school operations and/or regional operations.

As a person, you have a good ability to plan and structure your work. You enjoy driving tasks forward independently, while functioning well in teams where support and collaboration are a natural part. Furthermore, you are receptive, empathetic, and have an easy time collaborating with others. You have a good ability to meet relevant persons and partners in a respectful and receptive manner.

The position requires that you have good knowledge of Swedish, both in speech and writing.
