We are looking for a General Practitioner who wants to take ownership of their patients over time – and sees this as an advantage, not a burden.

WHAT CONTINUITY MEANS IN PRACTICE FOR US

Since we introduced PAL/PAS teams and a continuity-based list model, we have seen concrete changes: fewer sick leaves, lower prescription rates of narcotics, and – most importantly – patients who know both their doctor and their nurse. We are proud of this.

We do not date your list – you build it based on continuity and adapted care intensity. If you have mentoring duties, the list size decreases proportionally and follows your employment grade. This entails significant personal responsibility, and it requires a doctor who wants it.

ABOUT THE COMPOSITION OF THE LIST

We have 1,800 listed patients per full-time doctor. The National Board of Health and Welfare talks about 1,100 but does not take care intensity into account – and it is this intensity that determines what the day looks like. At our clinic, 16% of patients seek care less often than once every three years. We factor this in when setting the list size.

HOW WE WORK

PAL/PAS team – you and a dedicated nurse share responsibility for the same patient list and plan together via an annual wheel. Well-controlled chronic patients are followed by district nurses with structured protocols. Each week, time is allocated for home care rounds and home visits. The standard time per patient visit is 30 minutes; you assess home visits during round time yourself.

WE ARE BUILDING ELDERLY CARE

We are currently building an elderly care clinic with proactive identification of frail elderly, in collaboration with rehabilitation services and the municipality – an evidence-based improvement project. You will be part of the construction from the start.

YOUR ASSIGNMENT

• Clinic with own patient list and 30-minute slots
• Home care rounds and home visits with the responsible nurse
• Participation in SÄBO (supported housing) medical care
• Treatment and annual checks according to the annual wheel
• Emergency and sub-acute clinic in a rotating schedule
• Active role in the development of the elderly care clinic

THE TEAM

Three specialists, three ST-doctors (residents), and BT- and AT-doctors (specialists in training) for parts of the year. Eight nurses, the majority of whom are district nurses with long experience. Mentoring is scheduled and time-compensated. We also have a mental health team with a psychologist, psychotherapist, CBT therapist, care coordinator, and rehabilitation coordinator.

WE ARE LOOKING FOR YOU WHO

• Is a specialist in general practice with Swedish licensure
• Communicates fluently in Swedish
• Wants to take ownership of your patients over time
• Has a genuine interest in the care of the elderly and multi-morbid patients
• Thrives in a team and wants to help build – not just fill a position

QUALIFICATIONS THAT ARE AN ADVANTAGE

• Experience from home healthcare, SÄBO, or geriatric assessment
• Interest in frailty assessment or Clinical Frailty Scale
• Experience mentoring ST/BT/AT-doctors

CONDITIONS
