Do you want to help shape the future of dementia care in Höör Municipality? We are now looking for an engaged and knowledgeable nurse with specialist training in dementia for our newly established dementia team – a team with high competence and a central role in the municipality's development work.
About the Dementia Team
The Dementia Team will serve as the municipality's specialist and support resource in the field of dementia, consisting of a dementia nurse and a care assistant with specialist competence in dementia. Additionally, occupational therapists, physiotherapists, and care coordinators will collaborate closely with the Dementia Team. The team's mission is to ensure that people with dementia receive early support, coordinated interventions, and safe, evidence-based care. We work consultatively, supervisory, and person-centered – always with the individual's needs in focus.
The team leads the municipality's dementia work in line with the National Board of Health and Welfare's guidelines and the national dementia strategy. We collaborate with home care services, special housing facilities, needs assessment officers, licensed personnel, and MAS/MAR.

Job Responsibilities
Your tasks
As a nurse in the Dementia Team, you will have a varied and developing role where you will work with:
Individual and Relative Support
- Conduct home visits and assessments
- Identify early signs of cognitive impairment
- Support and inform relatives throughout the disease process
- Collaborate in SIP (Individualized Service Plan)
Specialist Assessments
- Assess symptom patterns including BPSD (Behavioral and Psychological Symptoms of Dementia)
- Conduct nursing assessments within the municipality's HSL responsibility
- Suggest individualized interventions
Supervision and Competence Development
- Supervise and educate staff in person-centered working methods
- Work with the BPSD register
- Participate in quality and improvement work
Coordination and Development
- Ensure that dementia care follows national guidelines
- Develop routines and working methods
- Participate in cross-professional collaboration
Working Methods
You will become an important part of a team that works:
- Person-centered, based on life stories and individual needs
- Consultative and supervisory, especially in complex cases
- Structured according to the BPSD model: mapping, analysis, planning, and follow-up
- Coordinated and cross-professionally together with the municipality's various operations
- Preventive and health-promoting, focusing on fall risk, nutrition, oral health, welfare technology, social participation, and security
Qualifications
We are looking for you who are:
- A licensed nurse with specialist competence in dementia care
- Good collaborative and pedagogical skills
- Experienced in municipal care and confident in working independently and consultatively
- A good communicator with the ability to build trust and collaborate across operational boundaries
- Committed to development work and accustomed to working in a structured manner
Merits include experience with BPSD work, supervision, and educational initiatives.

Salary: Monthly salary.
Employment type: Permanent position.
Duration: Permanent, start date: By agreement.
Working hours: Daytime. The position involves full-time work Monday-Friday.
The position will be filled subject to necessary political decisions being made and provided it does not need to be reserved for internal use.
