Social Worker for Follow-up Group – Tierp Municipality

Are you a social worker and want to work with children, young people, and their families? We are looking for a new colleague for our follow-up group who wants to further develop the work and ensure that children and young people live in a safe living situation.

Job Responsibilities

As a social secretary in the follow-up group, you work in various ways to ensure that all children in the municipality are doing well and growing up in safe conditions. You do this, among other things, by ensuring that interventions within social services are of good quality. This is done through regular follow-ups of interventions together with the child, guardians, and the provider of the intervention.

In the role of a follow-up officer with us, relationship building is important, and you follow up both outpatient interventions and placements in, for example, foster homes and residential care facilities (HVB), with the target group aged 0-21 years and their families. The follow-up is an ongoing process that both the social services board and the care provider must document continuously. It provides a basis for considerations and reviews of the care.

The follow-up of care aims to ensure that the child is doing well and receiving the right help. The goals formulated in the child's care plan and implementation plan are important starting points when social services follow up the care. During the follow-up, new needs can be identified and any potential problems in the home where the child resides can be highlighted.

The work consists, in addition to follow-up, of planning, collaboration, case processing, and documentation, decision-making, and other authority exercises, as well as to some extent investigation.

Qualifications

We are looking for you who are a social worker and want to contribute to good meetings with children and young people as well as their families. Experience in authority work, good knowledge of BBIC and Signs of Safety, thorough knowledge of the Social Services Act, and good documentation skills are meritorious. We place great emphasis on personal suitability.

To thrive with us and in the job, you are curious, have an easy time building relationships, and have an interest in people. You are structured and enjoy collaborating, but also have the ability to work independently. You have knowledge of children's needs and are passionate about contributing to good and quality work.

Travel and home visits are part of the position, so you need to have a B-driver's license for manual cars.
