We offer you a developing and varied combined position within home care and the close care team in Söderköping. The role suits you if you are experienced in your profession and driven by working close to the patient with a holistic perspective, where you can work both independently and in a team.

The close care team is part of the initiative Elderly Health and Well-being, where the focus is on creating a more cohesive, proactive, and person-centered care for our most vulnerable elderly. The work in the team is characterized by development, innovative thinking, and close cooperation between different professions and healthcare providers.

In combination with home care, you get a daily life with great variation, where you meet patients in their home environment and are responsible for planning, implementation, and follow-up of care measures. Within home care, there are also nursing interventions within our LSS (Act on Support and Service for Persons with Certain Functional Impairments) activities. The work involves at times a high pace and complex situations, which places demands on prioritization skills, flexibility, and confidence in your professional role.

What is specific to the position is the switch between independent work in home care and the team-based, more coordinating work in the close care team. You have close cooperation with colleagues within both municipal health and medical care, care staff, rehabilitation personnel, as well as doctors and other external healthcare providers.

Job Description

As a nurse/registered nurse in this combined position, you work both within home care and in the close care team.

The work in home care includes:

  • Assessment, planning, and follow-up of health and medical care measures in the patient's home
  • Medication management, medical interventions, and medication delegation
  • Collaboration with care staff and other healthcare providers
  • Documentation according to current legislation

The work in the close care team involves:

  • Proactive and person-centered work with a focus on vulnerable elderly
  • Coordination of measures around the individual
  • In-depth medical assessments
  • Collaboration across organizational boundaries
  • Participation in development and improvement work

The position entails significant personal responsibility and requires good structure, the ability to prioritize, and the ability to handle complex care situations.

The position includes weekend duty, which is currently every sixth weekend.

Qualifications

Requirements:

  • Registered Nurse (RN) or District Nurse
  • Several years of experience within municipal health and medical care
  • Good knowledge of Swedish, both in speech and writing
  • Valid driver's license (Category B)

Meritorious:

  • Experience working with the elderly and/or patients with multiple illnesses
  • Experience of collaboration between different healthcare providers
  • Experience in improvement or development work
  • Experience with the activity system Lifecare

Personal Competencies:

You are quality-conscious and work structured with a focus on patient safety. You have good judgment and can make well-founded decisions in complex situations. You are flexible and have the ability to adapt to changed needs in the operations. The role requires a high degree of independence combined with very good teamwork skills and a good ability to prioritize under high workload.
