We are now looking for a Social Coordinator for the Emergency Clinic, which includes both the Emergency Department and the Acute Care Unit (AVA). The Emergency Department

At the Emergency Department, we treat patients with acute orthopedic, medical, and surgical conditions. We work in close cooperation with the Pediatric Emergency Department and Emergency Radiology, creating good conditions for efficient and secure patient care. Teamwork is central to our operations. Nursing assistants, nurses, and doctors work together around the patient, contributing their respective competencies. The work is varied, sometimes intense, and characterized by non-hierarchical collaboration.

Approximately 170 employees work here, consisting of doctors, nurses, nursing assistants, and care administrators.

The Acute Care Unit (AVA)

At AVA, we meet patients with various medical diagnoses of varying severity. Admissions and discharges, acute nursing care, medical treatment, and monitoring take place here around the clock. AVA has 16 care beds distributed across three care teams, one of which provides Intermediate Care (IMA). Patients are usually cared for for 1–2 days before they are either discharged or transferred to another ward.

AVA also has four intermediate care beds for patients who are too ill for a regular care ward but do not require intensive care.

Approximately 50 employees — nursing assistants, nurses, and personnel coordinators — work on the ward in an operation with high competence, strong team spirit, and a clear patient focus. We have an open communication climate with daily reflection and feedback where all employees participate.

Job Description

As a Social Coordinator, you are responsible for ensuring that patients at the Emergency Department and AVA have a safe and functional transition to their homes or municipal services. You work closely with healthcare staff, municipal actors, and relatives to create a cohesive care chain.

The role is currently being implemented and may be adjusted over time, both regarding working hours and staffing needs. Currently, staffing is planned for weekdays during day and evening shifts, as well as daytime during weekends. The work with social coordination is estimated to cover approximately 70–80% of the position, while the remaining part involves clinical work within the Emergency Clinic.

Responsibilities

  • Identify patients in need of social coordination
  • Coordinate contacts with the municipality, home care services, home nursing, and rehabilitation
  • Participate in rounds at the Acute Care Unit
  • Document actions in the Cosmic system
  • Plan and coordinate safe discharge, including booking transport if needed
  • Maintain continuous communication with staff at AVA, the Emergency Department, and the care place coordinator
  • Work in care teams and contribute with targeted interventions when the situation requires it
  • Handle authority contacts through collaboration with social workers when the patient lacks decisions or needs extended services

A central part of the role is to work systematically with the Clinical Frailty Scale (CFS). You follow up on assessments and ensure that interventions are adapted to the patient's level of frailty.

About you

We are looking for someone who:

  • Is a nursing assistant, or has other education that the employer deems relevant
  • Has experience from healthcare (meritorious, preferably emergency care, but not a requirement)
  • Has documentation experience and confidence in working in the medical record system Cosmic
  • Has commitment, a sense of responsibility, and a desire to drive development
  • Is communicative and enjoys collaboration, especially regarding frail elderly patients
  • Is flexible and can quickly adapt to changing situations
  • Is sensitive and confident in dialogue with colleagues, patients, relatives, and authority contacts
  • Has good knowledge of Swedish in both speech and writing
