To Doktor.se Alnö Primary Health Care Center, we are now looking for an engaged diabetes nurse who wants to become part of a wonderful team and contribute to developing the primary care of the future together with us.

About the role

As a diabetes nurse at Doktor.se Alnö Primary Health Care Center, you work based on patient needs and evidence. You are professional in your approach and prioritize putting the patient first.

Your responsibilities include:

• Standard clinic duties
• Advising via phone and chat
• Triage and handling of acute patient flows
• Education, support, and counseling for patients with diabetes
• Contributing to the development of our diabetes care and improvement work
• Participating in the development of primary care by implementing digital-physical processes

About you

To thrive in this role, you are confident in your professional role and appreciate working in a changing environment where you have the chance to influence and contribute with innovative thinking. There are significant development opportunities in this role. We are looking for you with the following qualifications:

• Registered District Nurse or Nurse
• Specialist competence or experience in diabetes care
• Good knowledge of Swedish, both spoken and written
• Knowledge of the medical record system Cosmic
• Experience in diabetes is an advantage

• Desired start date is as soon as possible, but we naturally take your notice period and preferences into account.
• The position is a permanent full-time employment with an initial probationary period.
