Nurse/Community Nurse for Home Care in Region Gävleborg

Viraliv is now looking for an engaged and experienced registered nurse or community nurse for an exciting assignment within home care in Region Gävleborg.

About the Assignment

As a nurse in home care, you are responsible for planning, implementing, and following up on healthcare measures for patients in their own homes. Duties include, among other things:

• Medical assessments and nursing interventions
• Medication management
• Scheduled and emergency home visits
• Telephone advice
• Coordination with doctors, rehabilitation staff, and other healthcare providers
• Documentation in the Combine medical record system

You work independently with great personal responsibility while being part of an experienced and supportive team.

We are looking for you who

Requirements:

• Are a registered nurse or community nurse
• Have at least 3 years of professional experience
• Hold a valid driver's license (Class B)
• Have access to your own car for commuting to and from work
• Have good proficiency in Swedish, both spoken and written

Merits:

• Experience in home care
• Experience in municipal healthcare
• Experience with the Combine medical record system
