As a Social Secretary/Case Manager, you have an important job where you create value for the people around you every day. Do you have a strong sense of responsibility and focus on individuals' specific needs? Then you might be the person we are looking for. The unit is part of the main department for care and social services and belongs to the Individual and Family Care (IFO) operations.

Responsibilities

As a Social Secretary specializing in elderly care, you work from a holistic perspective and meet users with the starting point of strengthening their own abilities and the individual's continued independence. Your main tasks are to investigate and assess needs, follow up on the measures you approve, and ensure that the measure meets the individual's needs regarding, among other things, home help services, day activities, residential care, short-term stays, and special housing primarily for elderly persons.

You handle cases within current legislation (Social Services Act), current legal situations, and established guidelines. We work with continuous case management in groups. Case handling and documentation take place in the business system Life Care. We base our assessments on the IBIC model to describe, assess, and follow up on the individual's needs.

Your duties also include participating in network meetings, writing statements, being responsible for phone hours, and actively participating in ongoing development work. The work requires secure case handling where you, as the case manager, must ensure that the applicant is involved and able to express their views. You have close cooperation with the individual, relatives, providers, colleagues, as well as other professions and authorities. The work is focused on quality and legal security with the user in focus.

Qualifications

We are looking for you who have a degree in Social Work (Social Worker).

Experience from previous work within social services is considered meritorious.

You have an interest in working with people in different life stages. You are independent, have good cooperation skills, are responsible, and have the ability to create trust-building meetings. As a person, you are used to taking personal responsibility for your workday, planning and completing your tasks in a legally secure and efficient manner. Your tasks require that you are flexible, see opportunities in changes, and are meticulous. You can express yourself clearly and well-formulated in speech and writing.

The Social Services in Sollefteå strive to work systematically, and you should have the ability to see and work from people's own strengths and resources.

Great importance is attached to personal suitability.

A B-class driver's license is a requirement.
