About the workplace

The home nursing unit at Fyrislundsgatan 68 provides municipal home care to individuals aged 65 and older. We work in a person-centered manner, ensuring that every patient receives safe and secure care in their home environment.

The workplace serves as the base for our nurses and other licensed personnel. Here, we plan the day, document, hold meetings, and collaborate with other professions. The majority of the work takes place in the patients' homes, in close collaboration with home help services, social workers, doctors, and rehabilitation staff.

The work is performed during daytime hours, both on-site and at patients' homes.

Your responsibilities

As a nurse in home nursing, you work independently, supported by knowledgeable colleagues and close cross-professional collaboration. You are responsible for planning, implementing, and following up on health and medical care interventions in the patients' homes.

You will primarily work with:

• assessment, planning, implementation, and follow-up of nursing interventions
• medical interventions such as medication management, injections, infusions, wound dressings, and sample collection
• follow-up of patients' health status and nursing needs
• coordination and collaboration with doctors, rehabilitation staff, social workers, and home help services
• supervision and delegation to nursing staff
• documentation in accordance with current laws and guidelines
• participation in quality work and development initiatives

Qualifications

We are looking for someone who:

• is a licensed nurse according to the National Board of Health and Welfare
• has knowledge of current health and medical care legislation
• has knowledge of medication management according to current regulations
• has experience working according to national and local guidelines and routines
• has basic computer skills and experience with documentation in digital medical record systems
• has a valid B-driver's license
• has good knowledge of Swedish, both spoken and written

Merits

Personal qualities

In the role of a nurse in home nursing, you work independently, take responsibility for your tasks, and make decisions even in changing situations. You are flexible and see opportunities when conditions change. With good collaboration skills, you create security in meetings with patients, colleagues, and partners. You also have pedagogical skills and can adapt your way of guiding and delegating. With a clear holistic view, you make decisions that benefit both the patient and the operation.
